How the insurance process works

    Before your appointment we contact your carrier to confirm coverage, deductible, and remaining annual maximum.

    Written estimate

    You receive a written estimate showing what your insurance will likely cover and what you will likely owe.

    Treatment + claim filing

    You pay your portion at the visit. We file the claim directly with your carrier.

    Refund if applicable

    If the carrier reimburses more than expected, we refund the difference back to you.

Frequently Asked Questions

Is Hope Endodontics in-network with my plan?
We participate with most major PPO plans. The most common in-network carriers in our office are listed above. If yours is not listed, we still file as out-of-network and many patients pay similar out-of-pocket because endodontics is typically reimbursed at the same percentage. Call (480) 943-1900 and we will verify your benefits before the appointment.
Do you accept HMO or capitation plans?
No. Most endodontic specialty offices in the Phoenix metro do not contract as HMO/DMO providers because the case complexity is incompatible with capitation reimbursement. If you have an HMO plan, your general dentist may need to refer in-network, or you can pay out-of-pocket and we will provide a detailed receipt.
Will my insurance cover a root canal?
Most PPO plans cover root canal therapy at 50–80%, after a deductible. Coverage usually depends on the tooth (anterior vs molar), whether it is a first-time procedure or retreatment, and any annual-maximum that has been used. Our team verifies all of this before your appointment.
How does the insurance process work?
Before the appointment we verify your benefits and provide a written estimate showing what your insurance is likely to cover and what you will likely owe. After the visit we file the claim directly with your insurance. You pay your portion at the visit; if there is a refund after the claim processes, we send it back to you.
What if I do not have insurance?
We accept HSAs, FSAs, all major credit cards, CareCredit financing, and offer in-house split-pay for some patients. See our financing page for details.
